Course Directors may purchase the mailing information for certified occupational hearing conservationists by using this form. You must indicate what date range you are selecting (e.g., a technician who needs to recertify in the next 6 months or over the next 12 months, or all currently certified over a 5 year range); what state or neighboring states OR what zip code ranges (e.g., the first 3 digits of a geographic area - Milwaukee downtown = 532).

These names are for a one-time use only and may be ordered as a self-adhesive label or electronic textfile. Please note: lists do not include email addresses, these are for mailing addresses only. Email addresses are not sold by CAOHC as an agreement to our certificate holders.
